The Urdu word for condolence is تعزیت (taziyat). To say it to someone you can use میں آپ کے غم میں شریک ہوں (main aap ke gham mein shareek hoon), "I share your grief". It is also called اللہ آپ کو صبر دے (Allah aap ko sabr de).
